Putty - Can't Connect

I'm hoping someone has done this before and knows what to do.
I'm using Delphi for my APP and need to access a MYSql db on my ISP server. As my IP address changes, I need to use Putty to connect my app with the server. My ISP only allows users with pre-set IP address to connect to the db

I can login to the server with Putty but I cannot connect to the DB. I'm not an internet expert. Any suggestions on how to go forward, or any other option I can use to get my DB working ?

After 30 minute on the phone to MY ISP, we got it working. It all has to do with putting the correct login names in the correct places. Not that easy.
