Need an example of web.config URGENTLY

Another day spent trying to get a most simple application with Roles and Memberships included deployed just so I can test things.
This is a constant problem. I've tried several times over the last year or so to no avail.
Has anyone come up with a simple wizard or check list as how to proceed.
Any assistance is most welcome.
