We tested an application with your config file. Everything works.
Use "Full" trust level. If it works, try to find permission which blocks work on your end.

Just to be sure; Does the hosting provider have to install the Corelab MySQLDirect for .NET on the server, or is the xcopy deployment where the required dll's reside in the /bin folder enough?

"xcopy deployment" should be enough.
