Entity framework insert fails during stress testing.

I am getting this exception ONLY during stress testing. I am using Direct mode w/o any pooling info. in the conn. string (assuming default values). I could increase the MaxpoolSize but the default '100' should be plenty for our scenario. Do you have any ideas how we can track the connection usage? can DbMonitor help ?
